Policies and Procedures

Important things to note:

  • All trips are received directly from medical facilities. Urgent Transportation trips are not scheduled through Veyo.
  • All claims need to be submitted through MMIS, Veyo is unable to submit claims on your behalf.

 

Vehicle Requirements

All vehicles must meet all Americans with Disabilities Act (ADA) standards and all wheelchair accessible vehicles must contain approved wheelchair straps. The vehicle requirements are:

Exterior of Vehicle-

  • External signage that includes PUC identification number on both sides of the vehicle and is visible from 50 feet away
  • External signage displaying the company name and phone number in at least 3-inch letters
  • Exterior is free of broken mirrors or windows, excessive grime, major dents or paint damage
  • At least 2 exterior rear-view mirrors, one on each side of the vehicle
  • Full size vans or ambulances must be equipped with retractable step, fixed sideboard or and approved step stool

Interior of Vehicle-

  • Adequately functioning heating and air conditioning systems
  • Functioning, clean and accessible seat belts for all seating positions
  • Accurate and operating speedometer and odometer
  • Interior is clean and free of torn upholstery, torn or damaged floor and ceiling covering, damaged seats and sharp edges
  • Interior mirror for monitoring passenger compartment
  • Functioning interior light within passenger compartment
  • Visible interior signage stating:
    NO SMOKING
    ALL PASSENGERS MUST USE SEAT BELTS
  • Vehicle must have a vehicle packet containing vehicle registration, insurance card, and accident procedure forms
  • Fully equipped first aid kit
  • Current map with enough detail to locate clients and medical providers
  • GPS device
  • Interior must have sidewall padding and adequate anti-skid flooring or carpeting

 

Driver Requirements

For a driver to be eligible for Urgent Transportation services, they must have the following documentation on file with Veyo:

  • Completed Driver Applicant Information Form
  • Completed Provider Driver Information Sheet outlining the training that the driver has undergone
    • Must include defensive driving techniques, cultural and disability sensitivity training, passenger assistance techniques, first aid, general customer service, and wheelchair securement and lift operation (when applicable)
  • Photocopy of a valid Colorado Driver’s License
    • Driver must be over 20 years old
  • Motor Vehicle Records
    • Must show at least 3 years of driving experience in United States
      • If 3 years’ experience is not available on Colorado MVR, you must provide the additional state(s)’ MVR so we have a 3-year history in the United States
    • Driver must not have more than 4 points in the past 12 months and no more than 6 points in the past 3 years against their license for moving violations
    • Driver should not have their license suspended or revoked in past 5 years
  • Drug Test – 9-11 Panel Test
    • Must have tested for Benzodiazepines, Propoxyphene, Cocaine, Barbiturates, Opiates, Methaqualone, Methadone, Phencyclidine, Amphetamines, and Marijuana
    • Drug testing will be conducted annually
  • Proof of Fingerprint Submission (IdentoGO)
    • All drivers will obtain pre-employment fingerprinting and FBI background clearance checks (Veyo is not responsible for these costs). Drivers will initiate this process and have the results sent to Veyo
    • Add Veyo’s Account Code when completing the fingerprint submission – Account Code to note is CONCJ5411
    • Driver cannot have prior convictions for substance abuse, sexual abuse or violent crime
    • Drivers must comply with the criminal requirements in 4 CCR 723-6. For more information on these requirements, click here

 

Accident and Incident Procedures

If you are involved in an accident or have an incident where a member is injured during an Urgent Transportation trip, this needs to be reported to Veyo immediately. If this occurs, please complete the following form:
